Terms of Service
Last updated: 2026-05-22
These Terms of Service ("Terms") govern your use of this website (ccdilab.com and its subpages, the "Website"), which is operated by Consolacion Community Doctors, Inc. ("CCDI", "we", "us", "our").
By using the Website, you agree to these Terms. If you do not agree, please do not use the Website.
1. About this Website and CCDI
CCDI is a diagnostic laboratory and medical clinic operating in Consolacion, Cebu, Philippines, continuously since 2003. We are duly registered and accredited under Philippine law.
This Website is an informational and corporate-sales tool. It allows you to:
- learn about our services for individual walk-in patients and for corporate clients;
- find our clinic location, hours, and contact information;
- book a 15-minute discovery call with our team to discuss corporate healthcare arrangements for your company.
The Website is not a telemedicine platform. We do not offer medical consultations, diagnoses, prescriptions, or treatment through this Website.
2. Eligibility
You may use this Website if you are at least 18 years old and capable of entering into a binding agreement under Philippine law. If you are booking a corporate discovery call on behalf of a company, you confirm that you are authorized by that company to do so.
3. Scope of the Corporate Discovery Call
A "corporate discovery call" booked through this Website is a no-obligation conversation, normally 15 minutes, between you (or a representative of your company) and a member of our team. Its purpose is to:
- understand your company's healthcare needs (e.g., pre-employment medicals, annual physicals, drug testing, laboratory services);
- describe the relevant CCDI services and pricing tiers;
- answer your questions about how an engagement would work.
The corporate discovery call is:
- not a medical consultation;
- not an offer to provide medical advice, diagnosis, or treatment;
- not a binding contract for services. Any actual engagement between CCDI and a company is documented in a separate written agreement that both parties sign.
We may, at our discretion, decline to take a booking, reschedule, or end a call where the booking falls outside the intended audience (for example, a request for an individual medical consultation that would be better served at the clinic).
4. Medical Disclaimer (IMPORTANT — please read)
Nothing on this Website is medical advice. The content of this Website is for general information only.
- Do not use this Website as a substitute for in-person consultation with a licensed physician.
- Do not rely on any information on this Website to diagnose, treat, prevent, or cure any disease or health condition.
- If you are experiencing a medical emergency, call your local emergency number or visit the nearest emergency department immediately. Do not use this Website.
If you are a walk-in patient who needs a consultation, please visit the clinic at our address during open hours, or call us at the numbers listed on the Website.
5. Information you provide
When you book a discovery call, send us a message, or contact us by phone, Viber, or email, you provide us personal information. Our use of that information is governed by our Privacy Policy. By using the Website, you confirm that the information you provide is accurate, current, and complete.
